2. Information About Services

This website provides general information about CEBHS services, which may include mental health support, substance use recovery support, peer support, reentry and reintegration programs, life skills and employment support, family support services, and related behavioral health or community-based support services.

Website content is provided for general informational purposes only. It is not intended to diagnose, treat, cure, or prevent any condition, and it does not create a provider-client relationship by itself.

Services, availability, eligibility, appointment formats, and program details may change at any time. Final service details will be confirmed directly by CEBHS.

3. Not for Emergencies or Crisis Use

This website, contact form, and appointment request system are not intended for emergencies, crisis intervention, or immediate clinical response.

CEBHS does not guarantee that website messages or appointment requests will be reviewed immediately.

4. Appointment Requests

Appointment requests submitted through this website are requests only. Submitting a request does not mean that an appointment has been accepted, approved, or confirmed.

CEBHS may review the request and follow up to confirm availability, appointment format, eligibility, required documents, consent forms, payment or insurance information, and any other next steps.

You are responsible for providing accurate contact information. CEBHS is not responsible for delays caused by incomplete, inaccurate, or outdated information.

5. Privacy, HIPAA, and Confidentiality

CEBHS takes privacy and confidentiality seriously. Depending on the type of service, information shared with CEBHS may be protected under federal and Wisconsin privacy laws, including the Health Insurance Portability and Accountability Act, commonly known as HIPAA, Wisconsin laws governing treatment records, and, when applicable, laws governing substance use disorder records.

This Terms & Conditions page is not the same as a HIPAA Notice of Privacy Practices. CEBHS should maintain a separate Privacy Policy and, where required, a separate Notice of Privacy Practices explaining how protected health information may be used, disclosed, accessed, amended, and protected.

Website forms may not be appropriate for sharing highly sensitive information. Please avoid submitting unnecessary medical, mental health, substance use, legal, or emergency details through general website forms.

6. Substance Use Disorder Information

Some substance use disorder records may receive additional protections under federal law, including 42 CFR Part 2, depending on the nature of the program and services provided.

If applicable, CEBHS will handle substance use disorder records in accordance with applicable federal and state confidentiality requirements. Additional written consent or authorization may be required before certain information can be used or disclosed.

If there is a conflict between this website page and a required clinical, legal, privacy, or consent document provided directly by CEBHS, the more specific document will generally control.

8. Telehealth and Virtual Services

CEBHS may offer virtual, video, phone, or other remote service options when appropriate and available. Telehealth may not be suitable for every person, service, or situation.
